摘要：目的 探讨纤维支气管镜(纤支镜)吸痰术在危重症合并急性呼吸衰竭中的治疗效果.方法 分析25例患者床旁多导心电监护下,接受纤支镜39次吸痰、冲洗或注药治疗,观察术前、术后临床症状、经皮血氧饱和度、氧分压、二氧化碳分压及X线胸片.结果 慢性阻塞性肺疾病15例20次吸痰,10例胸腹、颅脑术后感染并呼吸衰竭19次吸痰,均可有效改善通气状况,降低通气阻力.结论 纤支镜用于机械通气患者气道管理,针对性强,疗效好. Abstract： Objective To investigate the effect on the sputum suction by broncho fibroscope for patients with acute respiratory failure. Methods Twenty-five patients received sputum suction and bronchial irrigation and local drug injection by brocho fibroscope for 39 times. The clinical symptom, pulse saturation oxygen, partial pressure of oxygen, partial pressure of carbon dioxide, X-ray were observed and compared before and after brocho fibroscope. Results Twenty times of sputum suction were performed in 15 patients with chronic obstructive pulmonary disease, and 19 times of sputum suction were performed in 10 patients after neurosurgery or thoracic-abdominal operation, higher ventilation and lower airway resistance were observed after sputum suction. Conclusions Airway regulation by broncho fibroscope for artificial airway patients was proved to be a pertinent and effective method.
