摘要：目的 讨论原发性阑尾肿瘤的诊断与治疗问题.方法 对1995年至2009年收治的 29例原发性阑尾肿瘤患者的临床资料进行回顾性分析,并结合国内外文献报道.结果 类癌14例,黏液性肿瘤11例,腺癌 4例,发病率0.39%.所有患者均经手术治疗,术前诊断率37.5%,术中诊断率仅 76.3%.结论 阑尾肿瘤缺乏特异表现,与阑尾炎症密切相关,诊断不清率极高.钡灌肠及B超有助于诊断,同时强调术中冰冻切片的重要性.手术为主要的治疗方法,术式选择是影响预后的关键因素. Abstract： Objective To study the diagnosis and treatment of primary tumors of the appendix.Methods Twenty-nine cases of primary tumors of the appendix encountered in our hospital from 1995 to 2008 were retrospectively analysed and relative literatures were reviewed. Results Among 29 cases, there were 14 cases of carcinoid,11 cases of mucinous tumor and 4 cases of adenocarcinoma. The morbidity was 0.39%.All cases underwent operation, the rate of diagnosis was 37.5% before operation, the rate of intraoperative diagnosis was only 76.3%. Conclusions Appendicular tumors lack particular presentation, which presents similar syptoms as appendicular inflammation.The rate of misdiagnosis is extremely high. Barium enema and ultrasonography are helpful for diagnosis. Intraoperative frozen section is very important for diagnosis. Operation is a principle therapy, correct operation method is important for improving the prognosis of appendicular tumor.
